Upon completion of the Veteran’s Service Card Application Form, you will be provided with a confirmation message.
Information: Checklist
What you will need:
- Service, Regimental, or Social Insurance Number. Your Social Insurance Number will not appear on your Veteran's Service Card.
- Year of military enrollment
- Digital photo
- Digital copy of valid government-issued identification. We accept:
- Provincial driver’s licence
- Provincial health card
- Aboriginal status card
- Passport
- NDI-21 (DND civilian card)
- Photo ID from another government department issued by a federal, provincial/territorial authority (or local equivalent abroad)
- Digital copy of certificate of service
